Q1. When you are having a hard time with diabetes do you inform your health care team? why or why not?

Q2. Do you  notify your health care team when your current treatment isn’t working?

Q3. If you aren’t satisfied with the type of care you are receiving from your health care team do you inform them?

Q4. How do you tell your health care team they are not meeting your needs? or addressing the issues that are important to you?

Q5. What reasons would you not be completely honest with your HCP? why or why not?

Q6. How can we open the lines of communications with our health care team?
